Since 1899, Concord Regional Visiting Nurse Association has been committed to meeting the health needs of families in the communities we serve. As the largest home health and hospice provider in New Hampshire, our not-for-profit, community based agency offers people of all ages a wide range of personalized services and programs, including home care, hospice, palliative care, personal home services, pediatric and maternal child health services, and wellness programming.

Concord Regional Visiting Nurse Association serves more than 8,000 (last FY) patients throughout Concord and 43 surrounding communities.

The mission of Concord Regional Visiting Nurse Association is to improve the health of the people and communities it serves by managing illness and promoting wellness through all stages of life.

Central New Hampshire VNA & Hospice

Formed through the merger of Community Health & Hospice in Laconia and the VNA & Hospice of Southern Carroll County in Wolfeboro, Central New Hampshire VNA and Hospice is the oldest and largest VNA in the New Hampshire Lakes Region, serving 47 towns.

The mission of Central New Hampshire VNA & Hospice is to promote dignity, independence, and well-being through the delivery of quality home health, hospice and community based care services.

Central New Hampshire VNA & Hospice offers in-home healthcare services including home care, rehabilitation services, pediatric care, education and support services for children and families, and a comprehensive, team-based hospice program. We are the only agency in the region to offer in-home pediatric health care and hospice services, as well as a special ‘Pet Peace of Mind’ pet care service for our patients on hospice to alleviate the worry of caring for their pets.
